First Visit
You will typically be referred to our office by your general dentist, or another referring specialist. Once he or she determines that an oral surgeon should perform the required procedure, they will complete a referral form and send it to our office. You should also receive a copy and should contact our office as soon as possible to schedule an appointment.
Your first visit to our office (consultation) will be to determine your course of treatment. Upon completion of an examination, a review of your x-rays and a detailed medical history , you will have an opportunity to discuss the proposed treatment, necessary anesthesia, and your recovery with the surgeon. It is important to us that you have a good understanding of the treatment we will provide.
Please assist us by providing the following information at the time of your consultation:
- Your surgical referral slip and any x-rays if applicable.
- A list of medications you are presently taking.
- If you have medical or dental insurance, bring the necessary completed forms. This will save time and allow us to help you process any claims.
Please Note:
All patients 18 years of age or younger must be accompanied by a parent or legal guardian at the consultation visit and the surgery appointment.
X-Rays
If your dentist or physician has taken x-rays, you may request that they forward them to our office. If there is not enough time, please pick them up and bring them to our office. If additional films are necessary, they can be taken at our facility.
